Acknowledgement of Risk
If you have difficulty downloading this form please contact us prior to your ski date and we will fax or email it to you. If you fill out the acknowledgement of risk prior to skiing with us please do not write the date in. You will fill in that portion upon arrival. We require the signatures of a parent or legal guardian for all skiers under the age of 18. You will fill out one of these forms each day you ski with us. If you plan on skiing multiple days and need a parent or guardian's signature it is a good idea to fill out a few of these in advance.
